Missing Miner Certificate Request
Miners who have misplaced a Pennsylvania certification should email:
RA-EPBMSREG@pa.gov and provide the following information:
- The subject line should read: Request for Certificate Copy
- In the body of the email, please provide the following:
- A. Name of certifications that you would like to receive a duplicate of
- B. First and last name
- C. Date of birth
- D. Last four numbers of your social security number
- E. Telephone number
