Amendments and Terminations
Applications for amendment of a license shall be filed in accordance with Chapter 217. 131 of Title 25 of the PA Code which incorporates 10 CFR 30.32 by reference and shall specify the respects in which the licensee desires its license to be amended and the grounds for the amendment.
Application for Radioactive Material License for the Possession and Use of Byproduct Material (DOC)
Requests for termination of a license shall be filed in accordance with Chapter 217. 131 of Title 25 of the PA Code which incorporates 10 CFR 30.36 by reference and shall include the Department of Environmental Protection Certificate for Disposition of Materials form which serves to certify material disposition.
Certificate for Disposition of Materials (DOC)
General licensed devices can only be transferred (for disposal or to obtain a replacement device) to a person holding a specific license, such as the device manufacturer or licensed waste broker. However, in the specific case of change of ownership where a general licensed device remains in use at a particular location, then the new owner will be the new general licensee upon completion of the associated transfer of control paperwork.
